I acknowledge that by submitting this electronic form, I am registering to play as an individual at Rivershore Sport Complex during the 2008 season. I agree that I am, among other things I am responsible for the following:

1.	Payment of all league fees owed to Rivershore Sports Complex.
2.	To maintain control myself both on and off the field and while at Rivershore Sports Complex.
3.	To sign a waiver / roster form prior to taking the field for the first time.
4.	Notify the park office in the event that I cannot be present for scheduled games that I have been assigned to. In the event that I am not present for two consecutive weeks, that I will be taken off the team with no refund allowed.
