I understand that by submitting this form for league registration, I am responsible for the above named team, as well as, but not limited to the following:
1. Payment of all league fees owed Rivershore Sports Complex, based upon the seasons I have just chosen above.  (League fees must be paid in full prior to teams being placed on the schedule for the 2008 season.)
2. Providing all information requested to include a complete team roster and waiver form which must be turned in to the office prior to my team taking the field and updating as players are added.
3. To keep control and discipline my team both on and off the field to the best of my ability
4. Notifying the park office in the event I cannot field a team and must forfeit games.  Notification must be made at least 24 hours prior to your scheduled game(s).
